USAC bulk upload spreadsheets. Awarded bidders will be requested to provide completed bulk upload templates upon request. All awarded bidders for Internal Connections will be REQUIRED to provide the bulk upload template for Internal Connections.
3. Telecommunications Reporting Worksheet — 499-A
All intrastate, interstate and international providers of telecommunications within the United States, with very limited exceptions, must file annually.
4. Form 498 ID — 498
Service Provider Identification Number and Contact Information Form is used to collect contact, remittance, and payment information for service providers who participate in the federal Universal Service Programs. Used to apply for SPIN.
5. FCC Registration Number — FCC RN
FCC Registration is required for participation in the Universal Services Programs.
6. Service Provider Annual Certification Form — 473
Annual certification form MUST be submitted in order for USAC payments to be made to service provider.
7. Service Provider Invoice Form — 474
Form used by service provider to submit invoices to USAC for discount amount.
